DO-IT-YOURSELF ASSISTANT

The Do-It-Yourself Assistant is intended as a self-help tool for abuse victims. Professionals and others who routinely fill out protective order petitions and orders should visit the Pro User page of this website.

Locate the domestic violence program nearest you for more information on how and where to get assistance with filing petitions for orders of protection.

Worksheets are designed to gather the information needed to fill out petitions and order forms. Many battered women's programs, victim's assistance programs, clerks of court's offices, and legal aid offices help victims fill out and file petitions for restraining orders. You can save time by filling out the worksheet online, printing it out, and taking it to the local entity that assists victims with filing petitions. 

To fill out worksheets 

  • Ensure Adobe Acrobat Reader 4.0 or better is installed on your computer. If not, it is free and available here.
  • To fill out the first page click in the fields marked first name, enter your first name and [TAB] to the next field. At the end of a page [TAB] moves you to the next page.
  • Complete check boxes with either a mouse click or pressing space bar.  Ensure that you check only yes or no to each question.
  • There are 5 pages of worksheets in the one document. To move between pages, use the arrows on the navigation tool bar.
  • To print the pages click on the printer icon on the navigation tool bar.
  • IMPORTANT! To clear your form, click the [Reset] box on page 1 before leaving.
